Save yourself some time!  Ensure you can answer "yes" to ALL the following questions before filling out an Online Application AND Company Input Form:

     1)  Does your company have significant operations in New York State?  This means:

          a) Your company is headquartered in New York State (or you will relocate to NYS prior to

              receiving funding)

          b) 75% of your workforce resides in NYS

     2)  Do you consider your company to be a high-tech start-up? 

          (We consider high-tech companies to be companies with patents, trade secrets,

          and/or IP.  Examples include software, medical devices, biotech, etc.)

     3)  Are you aware that Excell's typical investment is between $250K - $500K?

     4)  Will the current round of investment allow your company to achieve a significant

          milestone?

If you answered "yes" to ALL of the questions above, then please complete BOTH of the following:
